In 2006, The Trust contracted with external evaluators to conduct a formative assessment of HealthConnect in Our Community (HCiOC) in its first year of operation. The goal of this evaluation was to assess progress of the program to date, and to determine recommendations for changes in the program design and implementation. 2007 Formative Evaluation for HealthConnect in Our Schools: Final Report – September 2007
Upon implementation of the HealthConnect in Our Schools (HCiOS) program, The Trust contracted with an external evaluator to conduct a formative evaluation of the HCiOS program during the program’s first year of operation. The overall purpose of the formative evaluation was to help improve the HCiOS program, by understanding the program’s early implementation and documenting the program’s early results. 2009 HealthConnect in the Early Years Home Visitation Participant Survey
This voluntary home visitation program focuses on health prevention, education and promotion of positive behavior change for expectant teens and first-time mothers to improve maternal health, pregnancy outcomes, infant and toddler health and development. To enhance ongoing evaluation and provide guidance for how to further develop the initiative, The Children's Trust initiated a telephone survey with program consumers, including mothers who accepted and declined enrollment. The purpose of the brief telephone survey was to assess awareness and perceptions of HCiEY, experiences in the program, whether their needs were met, what specifically was helpful or not, overall satisfaction, and reasons for not enrolling or ending participation. In July and August 2009, The Children's Trust contracted with NORS Surveys, Inc. to conduct the satisfaction survey regarding HCiEY.
