Report your Summer Satisfaction Survey Results
Each provider offering summer camp will administer satisfaction surveys to all program participants and their parents/guardians. Providers must report their survey findings through the following online survey no later than September 15, 2010:

Satisfaction Surveys
Click on the survey documents below to access child, youth, and parent satisfaction surveys in English, Spanish, and Creole.  Providers may use these previously-released surveys from The Children’s Trust or alternate surveys of their choice to fulfill contractual requirements regarding the administration and analysis of satisfaction surveys.

Satisfaction Survey Data Entry Option
The documents below may be used to help providers record responses from The Children’s Trust satisfaction surveys.  The “Instructions for Data Entry” document guides providers through data entry templates for the child, youth, and parent surveys.  The actual templates are included in the Excel document, “Data Entry Spreadsheets.”  In contrast to years past, providers are not asked to submit data spreadsheets to The Children’s Trust.
