Image 1HealthConnect is a comprehensive, three-tier, quality-driven health initiative, spearheaded by The Children’s Trust, with the goal of having the healthiest children possible in Miami-Dade County. HealthConnect:
  • Connects children and families to needed services before health conditions require a trip to the emergency room.
  • Provides information and support through home visitations to all new and teen mothers from the prenatal period through the early years.
  • Uses multidisciplinary health teams in every school to provide needed health and mental health services.

HealthConnect In Our Schools LogoEach school will have a team of professionals including a nurse or nurse practitioner, a social worker and two health aides. Each team will serve two schools and will offer physical, mental and behavioral health services.

Unique to this component is a strategic financial partnership with Miami-Dade Public Schools and the Miami-Dade County Health Department, to bring quality health services to every school in Miami-Dade County by 2011.


Benefits of HealthConnect In Our Schools:
  • A coordinated level of school-based healthcare at every school
  • Expanded health screenings and assessments with access to follow-up care
  • Mental and behavioral health services to identify and solve student health and educational issues
  • Nutrition and health counseling
  • Better access to a regular primary-care physician
  • Primary health services for uninsured students with no other options or access to health care
  • Emergency first aid/CPR

HealthConnect In Our Community LogoCommunity health workers, strategically located throughout Miami-Dade County, work neighborhood-by-neighborhood to help children and families navigate the complicated route to quality health care and services.

Community health workers will work closely with health navigators. Navigators are trained to enroll children and families into healthcare
funding sources. Together, they collaborate to increase access to primary-care physicians, health insurance and other health services, regardless of race, ethnicity, origin of birth/nationality, immigration status or income.

HealthConnect In the Early Years LogoFamily coaches help guide expectant teens and first-time moms, infants and toddlers to the quality health care they require. Offering free pre- and post-natal home visits, family coaches ensure children and families have the best possible health outcomes.
